Paladin reserves the right to update or otherwise modify this Policy, as it deems to be necessary. When applicable, these modifications will be effective immediately and will be incorporated to this Policy upon notice thereof, which may be provided by any reasonable mean, including by displaying a notice on the Website landing page or at the beginning of the updated Policy. Your continued use of the Website following such notice will be deemed to constitute an acceptance of the updated Policy.
This Policy applies to all information collected by Paladin via the Website, including personal information, that is, identifiable information about you (“Personal information”) and electronic information. This Policy does not apply to third-party online resources to which Paladin’s Website may link, where Paladin does not control the content or the privacy practices of such resources.
II. THE PERSONAL INFORMATION WE COLLECT OR MAY COLLECT
The Website includes three sections: 1) for prescribers; 2) for pharmacists; and 3) for ICLUSIG patients. The Personal information that Paladin collects depends on the section of the Website you use. You represent and warrant that the Personal information you voluntarily provide is current and accurate.
To access the section of the Website intended for prescribers for more information on the CDP and/or to become certified prescribers, prescribers must be registered users of the Website. To register, prescribers are asked to provide their complete name, professional speciality, valid and current license number and email address. Prescribers will also be asked to create and confirm their password. On following visits, prescribers will be able to access the Website by simply entering their email address and password.
Prescribers who wish to become certified prescribers must complete the certification program offered on the Website (“Certification Program”) by successfully completing the Knowledge Assessment and filling out a prescriber registration form (the “Prescriber Registration Form”).
When completing the Prescriber Registration Form, prescribers will be asked to provide their complete name, professional specialty, license number, email address, and the name of the practice/facility where they work, its full address, phone and fax numbers. Prescribers may also enter an office contact should it be required in their situation. By entering this office’s contact information, prescribers represent that they are duly authorized to provide this office contact’s information. Finally, prescribers will be asked to confirm that they agree to the Prescriber Registration Form terms and conditions and to sign electronically by typing their complete name.
Should the prescriber fail the Knowledge Assessment, Paladin may collect the score that the prescriber obtained at each try and the number of failures should the prescriber fail more than once.
Pharmacists may access the Website by providing their province of practice and their valid and current professional license number.
3. ICLUSIG patients
ICLUSIG patients are not required to provide any personal information to access this section of the Website. However, Paladin may sometimes collect electronic information about the users when they visit the Website; please refer to the section below entitled “Electronic Information”.
